Incident investigations and internal audits
When something goes wrong, the goal is to find out why and stop it happening again, not to find someone to blame. We investigate incidents with recognized root cause methods and help you check your own program before an auditor or a regulator does.

Root cause investigations
We gather the evidence, reconstruct the sequence of events and look past the immediate cause to the conditions, systems and decisions behind it, using recognized methods including TapRooT. The findings are written so they hold up when others read them.
Corrective actions that fix the system
Each finding leads to a corrective action with an owner and a date, aimed at the system rather than the symptom, and a way to check later that it worked.
Internal audits and audit-readiness reviews
We review your program, records and field practice against the standard you will be measured on and tell you plainly where the gaps are. These are internal audits for your own improvement; we do not conduct external COR or SECOR certification audits.
Who calls us for this
- Employers after a serious incident, a near miss or a pattern of smaller ones
- Companies whose investigations keep ending at "worker error"
- Programs preparing for an external audit who want an honest internal one first
- Leaders who want an independent view of how their program is working
How it works
Step 1
Respond
We agree the scope quickly and secure what evidence remains.
Step 2
Investigate
Interviews, site review, documents and a timeline of what happened.
Step 3
Analyze
Root and contributing causes found with a recognized method.
Step 4
Report and follow up
A clear report, corrective actions, and a check that they were done.
What you have at the end
- An investigation report with root and contributing causes
- Corrective actions with owners and due dates
- An internal audit report with scored findings and priorities
- A plan to close the gaps before your external audit
Questions
Do you conduct COR or SECOR certification audits?
No. TruStar does not conduct external COR or SECOR audits. We do internal audits and audit-readiness reviews so you know where you stand before your certification audit.
What is TapRooT?
TapRooT is a recognized root cause analysis method that looks beyond the immediate cause to the system issues behind an incident. Our lead consultant is trained in it.
Can we run investigations ourselves afterward?
Yes. Many clients run their own investigations in TruStar Connect, which guides an investigator through the evidence and the causes and tracks the corrective actions.
